How Does Our Patient Intake Process Work?
We know you likely have been to several other doctors looking for relief or you may feel stress due to a misdiagnosis. It is our primary goal to be as comprehensive as possible in determining the exact cause of your neuropathy problem. In order to give us the best opportunity, our initial intake procedures occur on two separate visits. This ensures that we have the time to address all of the metabolic and neurologic concerns to your case. It also allows you plenty of time to meet with Dr. Veselak and his staff and discover if our clinic is right for you. We have helped patients from all over the country get their lives back. What Do the Two First Visits Entail?

If you choose to register to get scheduled for our examination, you will be offered an opportunity to schedule a 5 minute phone consultation with Dr. Veselak prior to coming in for your examination. This will allow you the opportunity to ask Dr. Veselak any questions regarding your case prior to your exam and ensure that our office is what you are looking for.
Dr. Veselak will review your intake forms, medical records, labwork and any x-rays or imaging studies that you may have brought with you. We encourage you to send us any previous lab testing prior to your first visit so that Dr. Veselak can review it with you. Dr. Veselak will perform a comprehensive, non-invasive neurological exam to discover how your neuropathy may be effecting the health of your brain. Are you aware that many of the symptoms you suffer from in neuropathy (brain fog, depression, anxiety, memory loss, chronic fatigue, etc.) are symptoms of changes occurring in the brain? With Dr. Veselak’s specialty in neurology he will be able to readily identify all areas of concern that are developing.
Dr. Veselak will let you know if he has decided to take your case. After reviewing your case history and performing your neurological exam Dr. Veselak will discuss his recommendations for future care and possible lab testing that may need to be performed. On this visit all fees for continuing care and lab testing will be discussed.
